Payment methods

Serenity Health Care Center accepts the following types of payment: 

  • Cash
  • Check
  • Spending Cards:  HSA/FSA FLEX cards

Wondering about which insurance we accept?

The only insurance that we will submit a claim to is Anthem/Blue Cross Blue Shield PPO Plans, as this is the only insurance that we are in-network.

If you have any other insurance, you will be considered a cash patient and will need to pay at the time of service. 

You may submit directly to your insurance carrier on your own behalf.  We are more than happy to supply you with a statement with your services, any appropriate CPT/ICD 10 codes, NPI, TAX ID and the amount paid, so you may submit to your insurance carrier directly.  

We also accept HSA/FSA Flex spending cards.  

No CARE Credit accepted.

TeleHealth and billing your insurance directly

We offer TeleHealth.  Your appointment can either be a phone consult or a ZOOM videoconference.  

If you are considering billing insurance on your own behalf directly, we may be able to provide you with a TeleHealth consult because they have no billable codes.  

If you are doing a phone consult or ZOOM and are not coming into the clinic, we will need:

  • A copy of your DL and Insurance Card
  • Sign off on payment consent and HIPPA Forms.

Even though we do not bill your insurance, we do require this information in case we need to do a prior authorization for testing or for prescriptions.

Medicare and Serenity

Unfortunately,  we are not a Medicare provider. Any Medicare patient would be self-pay.

Payment is due at the time of service.  

We ask that our Medicare patients sign off on a Medicare Waiver Form and an ABN Form stating they understand that we can not bill Medicare for services and that they understand that they can not bill Medicare on their own behalf either.

Use our patient portal service for quicker response times

Our Patient Portal facilitates communication with our office. The Patient Portal allows you to upload any documents for your provider, where you can send an email or a text to your provider, our front desk or other medical staff.

Use this convenient service to Request/Change an appointment.  

This method usually has a faster response time than calling the office!

What do appointment cancellations cost?

Cancellation Policy for New Patients

Serenity Health Care requires a credit card on file to hold your appointment.

Our policy requires that you provide us with notification that you are cancelling your appointment within 72 BUSINESS HOURS prior to your appointment.  

If your appointment is not rescheduled/cancelled within that 72 BUSINESS HOURS or if you do not show up for your appointment, you authorize Serenity to charge the credit card on file for a fee of $150.00.

Cancellation/No-Show Policy for current Patients

Serenity Health Care requires a 24 BUSINESS HOURS notice for any cancellations/rescheduled appointments.  If you cancel/reschedule/no-show, and it is less than 24 BUSINESS hours before your appointment, your credit card will be charged a $50 fee.  

For a lab appointment no-show, you will be assessed a $25.00 fee, charged to your credit card.
